                           TO:      Garnishee BANK OF AMERICA, N.A.

                           On   ________________,          2014,   Plaintiff   WASTE       MANAGEMENT             OF   Texas,    INC.

                  (“Waste    Management”)       made    affidavit   that  on  April   16,  2013,   in the  80th   District  Court    of

                  Harris   County,   Texas,   Plaintiff  Waste    Management      recovered    against   Anastasios    C.  Roth   (aka

                  Tasi Roth) a judgment in the amount of $96,463.00 for actual and exemplary damages, $251.00

                  in court costs, and post-judgment interest at 5% per annum, compounded annually, running from

                  April 16, 2013 (collectively the “Judgment”).            As of April 9, 2014, the amount of the Judgment

                  equals   $101,453.63.      The   Judgment     remains   due   and  unsatisfied.    To   the  affiant’s  knowledge

                  Anastasios    C.  Roth   has  no  property    in this state,  subject  to  execution,   sufficient   to satisfy  the

                  judgment.

                           Affiant   reasonably    believes   that BANK      OF   AMERICA,       N.A.   is indebted    to Anastasios

                  C. Roth or has effects belonging to         Anastasios    C.  Roth.    Accord

                  writ of garnishment against Anastasios C. Roth.

                           Therefore, BANK OF AMERICA, N.A. is commanded to appear before this Court, at the

                  Court House at 201 Caroline, Houston, Texas 77002 in Harris County, Texas, at 10 a.m. on the

                  Monday     immediately     following    the  expiration   of  twenty   (20)  days   from  the  date  of  service   of

                  debt or to deliver to him any effects,
