I, Dr. Ramesh Parchuri, MD licensed (MD033552E) have review the following materials in developing
my opinions in this case:
1 Medical records, medical bills, operative pictures, and radiology films from Sandra Choates
admission at Deborah Heart and lung Hospital from November 8, 2021, until October 20, 2021
2. Medical records, medical bills, and radiology films from Sandra Choate's treatment at Deborah
Heart and Lung Hospital from November
3 As discovery is ongoing, I reserve the right to supplement the materials I have reviewed in
rendering my opinions.
T understand that Plaintiffs and counsel team is currently obtaining additional records and bills from
several providers, including, but not limited to Deborah Heart and Lung Hospital. Capital Health,
Pennsylvania University Hospital
Therefore, I reserve the right to supplement this report to include opinions about those bills and that
treatment.
Tam a physician Dr. Ramesh Parchuri licensed to practice medicine in the state of Philadelphia,
Pennsylvania. I currently have 2 offices stationed in the Philadelphia area.
Ihave substantial experience as an M.D. including psychiatric services. Heart and Lung.
Tam familiar with the standard of medical care I currently evaluate and treat patients for many
conditions like Sandra Choate's conditions.
B. Summary of Records.
Sandra Choates presented to Deborah Heart and Lung Hospital on November 10, 2021, with signs and
symptoms consistent with congestive heart failure, this included a history of nausea and vomiting, and
right upper quadrant abdominal pain. Shortness of breath, chest pain, and weakness in the lower
extremities. CT scan Based upon this information, the patient was admitted to the hospital and scheduled
for observation and further testing. Along with a Swan-Ganz catheter procedure.

The surgery took place on November 10, 2021. Initially, the dissection was performed by a resident
Deborah Hospital, who admittedly had little to no experience performing the procedure. However, after
approximately 20 minutes of attempting, the operation was taken over by another doctor on the team.
Following her operation, the patient’s clinical condition rapidly deteriorated. She underwent numerous
radiological studies between November 10, 2021, and November 21, 2021, including, a HIDA scan, an
ERCP, and an MRI. The results of these tests in addition to the patient’s lab work overwhelmingly
confirmed that she had sustained a severe injury. In addition, Ms. Choates also was transferred to
Capital Health Hospital with Stroke like symptoms due to being over Medicated with severe mental and
physical trauma. She was then placed under a new physician's care team as follows.

Despite this evidence, the patient was then transferred to a higher level of care at Deborah Heart and
Lung Hospital until November 21, 2021. At that point, she required additional treatment, including
antibiotic therapy, until she was able to undergo more testing and treatment.
1 It is my opinion breaches of the standard of medical care substantially delayed Ms. Choates
treatment and proximately caused her to undergo additional treatment for months until she was
able to obtain another doctor.
I have reviewed all the medical records for the treatment Ms. Choates received at Devorah Heart
and Lung Hospital. Based upon a reasonable degree of medical certainty, treatment, including
administering the wrong medication breaches the standard of medical care.
As discovery is ongoing, I reserve the right to supplement and/or amend my opinions. | understand that
Plaintiff's counsel is currently obtaining additional records and bills from several providers, including,
but not limited to Deborah Heart and Lung Hospital. Capital Health Hospitals, Pennsylvania Hospital
Therefore, I reserve the right to supplement this report to include opinions about those bills and that
treatment.

WHAT YOU NEED TO KNOW:
Heart failure (HF) is a condition that does not allow your heart to fill or pump
properly. Not enough oxygen in your blood gets to your organs and tissues. HF
can occur in the right side, the left side, or both lower chambers of your heart. HF
is often caused by damage or injury to your heart. The damage may be caused by
heart attack, other heart conditions, or high blood pressure. HF is a long-term
condition that tends to get worse over time. It is important to manage your
health to improve your quality of life. HF can be worsened by heavy alcohol use,
smoking, diabetes that is not controlled, or obesity.

Go to cardiac rehab as directed: Cardiac rehab is a program run by specialists
who will help you safely strengthen your heart. The program includes exercise,
relaxation, stress management, and heart-healthy nutrition. Healthcare providers
will also make sure your medicines are helping to reduce your symptoms.
Manage your HF:
® Do not smoke. Nicotine and other chemicals in cigarettes and cigars can
cause lung damage and make HF difficult to manage. Ask your healthcare
provider for information if you currently smoke and need help to quit.
E-cigarettes or smokeless tobacco still contain nicotine. Talk to your
healthcare provider before you use these products.

© Weigh yourself every morning. Use the same scale, in the same spot. Do
this after you use the bathroom, but before you eat or drink anything. Wear
the same type of clothing. Do not wear shoes. Record your weight each day
so you will notice any sudden weight gain. Swelling and weight gain are
signs of fluid retention. If you are overweight, ask how to lose weight safely.

information about how to measure your blood pressure and heart rate
correctly. Ask what these numbers should be for you.

COPD. You will have fewer symptoms if you manage these health conditions

© Eat heart-healthy foods and limit sodium (salt). An easy way to do this
is to eat more fresh fruits and vegetables and fewer canned and processed
foods. Replace butter and margarine with heart-healthy oils such as olive oil
and canola oil. Other heart-healthy foods include walnuts, whole-grain
breads, low-fat dairy products, beans, and lean meats. Fatty fish such as
salmon and tuna are also heart healthy. Ask how much salt you can eat each
day. Do not use salt substitutes.
Drink liquids as directed. You may need to limit the amount of liquids you
drink if you retain fluid. Ask how much liquid to drink each day and which
liquids are best for you.
Stay active. If you are not active, your symptoms are likely to worsen
quickly. Walking, bicycling, and other types of physical activity help maintain
your strength and improve your mood. Physical activity also helps you
manage your weight. Work with your healthcare provider to create an
exercise plan that is right for you.
e Get vaccines as directed. Get a flu shot every year. You may also need the
pneumonia vaccine. The flu and pneumonia can be severe for a person who
has HF. Vaccines protect you from these infections.
Join a support group: Living with HF can be difficult. It may be helpful to talk
with others who have HF. You may learn how to better manage your condition or

COMMON USES: It is used to treat heart failure (weak heart).
BEFORE USING THIS MEDICINE: WHAT DO I NEED TO TELL MY DOCTOR BEFORE I TAKE THIS
DRUG? TELL YOUR DOCTOR: If you are allergic to this drug; any part of this drug; or any other
drugs, foods, or substances. Tell your doctor about the allergy and what signs you had, TELL
YOUR DOCTOR: If you have heart valve problems. TELL YOUR DOCTOR: If you have had a recent
heart attack. This is not a list of all drugs or health problems that interact with this drug. Tell your
doctor and pharmacist about all of your drugs (prescription or OTC, natural products, vitamins)
and health problems. You must check to make sure that it is safe for you to take this drug with all
of your drugs and health problems, Do not start, stop, or change the dose of any drug without
checking with your doctor.
HOW TO USE THIS MEDICINE: HOW IS THIS DRUG BEST TAKEN? Use this drug as ordered by
your doctor. Read all information given to you. Follow all instructions closely. It is given as an
infusion into a vein over a period of time. HOW DO I STORE AND/OR THROW OUT THIS DRUG? If
you need to store this drug at home, talk with your doctor, nurse, or pharmacist about how to
store it. WHAT DO I DO IF I MISS A DOSE? Call your doctor to find out what to do.
CAUTIONS: Tell all of your health care providers that you take this drug. This includes your
doctors, nurses, pharmacists, and dentists. Have blood work checked as you have been told by
the doctor. Talk with the doctor. Check blood pressure and heart rate as the doctor has told you.
Tell your doctor if you are pregnant, plan on getting pregnant, or are breast-feeding. You will
need to talk about the benefits and risks to you and the baby.
POSSIBLE SIDE EFFECTS: WHAT ARE SOME SIDE EFFECTS THAT I NEED TO CALL MY DOCTOR
ABOUT RIGHT AWAY? WARNING/CAUTION: Even though it may be rare, some people may have
very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical
help right away if you have any of the following signs or symptoms that may be related to a very
bad side effect: Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or
peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing,
swallowing, or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.

Very bad dizziness or passing out. Fast or abnormal heartbeat. WHAT ARE SOME OTHER SIDE
EFFECTS OF THIS DRUG? All drugs may cause side effects. However, many people have no side
effects or only have minor side effects. Call your doctor or get medical help if you have
any side
effects that bother you or do not go away. These are not all of the side effects that may occur. If
you have questions about side effects, call your doctor. Call your doctor for medical advice about
side effects. You may report side effects to the FDA at 1-800-332-1088. You may also report side
effects at https://www.fda.gov/medwatch.
OVERDOSE: If you think there has been an overdose, call your poison control center or get
medical care right away. Be ready to tell or show what was taken, how much, and when it
happened.
ADDITIONAL INFORMATION: If your symptoms or health problems do not get better or if they
become worse, call your doctor. Do not share your drugs with others and do not take anyone
else's drugs. Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets.
Throw away unused or expired drugs. Do not flush down a toilet or pour down a drain unless you
are told to do so. Check with your pharmacist if you have questions about the best way to throw
out drugs. There may be drug take-back programs in your area. Some drugs may have another
