COMES NOW the Plaintiff, Osceolasc Llc D.B.A. St Cloud Regional Medical Center by and
through the undersigned counsel and files this Motion for the Entry of Final Judgment Based on Default
in Stipulation\Settlement Agreement and would show unto the Court as follows:
1 That pursuant to the attached Affidavit, the Defendant, Adam T Cash, has failed to abide
by the terms of the Stipulation\Settlement Agreement entered into between the parties.
2. That the Plaintiff respectfully requests the Court enter a Final Judgment against the
Defendant pursuant to the terms of the said Stipulation\Settlement Agreement.
WHEREFORE the Plaintiff respectfully requests that this Court enter its Final Judgment in
