The Plaintiff is a Federally-Chartered Credit Union, licensed to conduct business in New York, with a principal place of business at 42 Union Street , Sidney, County of Delaware, State of NY. 2. Upon information and belief at all times hereinafter mentioned, the Defendant(s) is/are an individual(s), with a residence at 318 Jackson Summit Rd. E Mayfield, NY 12117. 3. That there is due and owing to the Plaintiff by the Defendant(s), the sum of $1,066.10 which includes principal of $641.10, plus accrued but unpaid interest as of October 26, 2021 in the sum of $0.00, . accumulated late charges and miscellaneous fees of $425.00, plus interest at the rate of 9.000% from October 26, 2021 by reason of the Defendant(s) default in the payment pursuant to the terms of a Checking and / or Savings Account dated December 12, 2014. 4. That although payment has been demanded of the Defendant(s) by the Plaintiff, such has been neglected and refused and/or denied.
