2021, along with the proposed Subpoena. The information is being subpoenaed for disclosure in
the course of a lawsuit filed by Plaintiff. The undersigned counsel herein certifies that the Plaintiff
has made a good faith attempt to provide written notice to the Defendants of Plaintiffs intent to
serve subpoenas to all nonparties and entities listed in the aforementioned Notice, which included
