OF NEW LONDON VS. : AT NEW LONDON SCOTT ROGERS ET AL : AUGUST 18, 2020 DISCLOSURE OF EXPERT Pursuant to Connecticut Practice Book Section 13-4, the Plaintiffs disclose the following as an expert witness whom the Plaintiff expects to call at the time of trial: Mohammad Pasha, M.D. Katherine Bangs, PA-C Kenneth Paonessa, M.D. Orthopedic Partners 82 New Park Road North Franklin, CT 06254 1. Field of Expertise and subject matter of which the expert is expected to testify: The above-referenced providers will provide opinions based upon their education, background and training in the field of Orthopedics, Physical Medicine and Rehabilitation as well as case-specific medical records, physical examination and treatment of the Plaintiff, Linda Doak. The above-referenced providers will testify as to the Plaintiff’s physical condition, the nature of her injuries, the nature of the medical treatment she received, the causation and opinions regarding future disabilities and physiological problems which the Plaintiff can expect to endure as a result of the injury which occurred on April 26, 2018 and gives rise to this cause of action. 2. Substance of facts and opinions 1 The facts and opinions to which the above-referenced providers will testify include their treatment of the Plaintiff, Linda Doak, as well as their review of various imaging results and tests. All of the above-referenced providers’ opinions are based on reasonable medical probability and are premised upon their education, training, experience and treatment of the Plaintiff. 3. Summary of Grounds: The grounds which will form the basis of the above-referenced providers’ opinions are their care and treatment of the Plaintiff, case-specific medical records, test results as well as their education, experience and relevant studies.
