1) The plaintiff, Jerzy R. Bielski, is an adult Massachusetts resident residing at 22 Elm
Circle, South Deerfield, Franklin County Massachusetts.
2) The defendant, James B. Crocker, is an adult Massachusetts resident residing at 27
Hillcrest Drive, Bernardston, Franklin County, Massachusetts.
3) North King Street, Northampton, MA is a public way.
4) On or about January 14, 2013, the defendant, agents, servants or employees or one acting
in his behalf, so negligently and carelessly operated a motor vehicle on, North King
Street, Northampton, MA that by reason there of an accident was caused.
5) As a result of the collision caused by the defendant, his agents, servants or employees the
plaintiff was injured, suffered pain of body and mind, required hospital and medical
treatment and could not carry out his usual activities for a long period of time, and has
incurred medical expenses in accordance with provisions of M.G.L. c .231, 6D.
WHEREFORE, the plaintiff demands judgment against the defendant; that damages be
established and plaintiff be awarded same, along with costs, interest and reasonable attorneys
fees.
JURY CLAIM, Plaintiff claims the right to trial by jury on all issues.
