of perjury, states that the allegations contained herein are true and are based upon personal
knowledge and belief.
1. My name is Adam J. Basch and | am an attorney licensed to practice in
the Commonwealth of Massachusetts.
2. | am familiar with and have personal knowledge regarding the allegations
contained in this affidavit.
3. | have been counsel for the Plaintiff since the institution of this litigation.
4. In connection with this litigation, the Plaintiff has incurred legal fees in the
sum of One Thousand and 00/100 Dollars ($1,000.00).
5. In connection with this litigation, the Plaintiff has incurred costs in the sum
of Four Hundred Fifty-Eight and 00/100 Dollars ($458.00).
6. M.G.L. Chapter 183A, §6(a) and §6(c) as amended by Chapter 400 of the
Acts of 1992, specifically provides for the recovery of attorney's fees and collection
costs.
