CONTINUING FINAL GARNISHMENT JUDGMENT
THIS CASE came on before the Court 3 parte upon PMI MORTGAGE
INSURANCE COMPANY's Motion for Entry of Continuing Final Garnishment Judgment.
The Court considered the Motion, reviewed the record, and being fully advised, it is
ORDERED and ADJUDGED that:
1. PMI MORTGAGE INSURANCE COMPANY'S Motion for entry of continuing
final garnishment judgment is hereby GRANTED.
2. PMI MORTGAGE INSURANCE COMPANY (hereinafter "PMI") shall have
and recover from Defendant, JOSE D. JAQUEZ, its post-judgment Court costs of $458.00
which sum shall bear interest at the rate of 4.75 percent (4.75%) per annum in accordance
with Florida Statute § 55.03(1), until paid or satisfied, for which let execution issue.
3. The Garnishee, PUBLIX SUPER MARKETS, INC., shall pay to

PMI amounts heretofore withheld pursuant to the Continuing Writ of Garnishment. In
addition, PUBLIX SUPER MARKETS, INC., shall hereafter continue to withhold from the
wages of the Defendant, JOSE D. JAQUEZ, and to pay to PMI, a sum not to exceed 25%
of the Defendant's disposable earning for each pay period, or the amount by which the
Defendant's disposable earnings for the pay period exceed 30 times the federal minimum
hourly wage, whichever is less, until the sum of $24,415.60, plus interest at the rate of
4.75% per annum from September 19, 201 4, plus costs in the amount of $458.00, is paid
or until further order of the Court. The Garnishee is directed to notify the Court and the
Plaintiff in writing upon the termination of the Defendant's employment.
4. Payments shall be made payable to Walter L. Sanders, P.A. Trust Account
and mailed to, 6936 W. Linebaugh Avenue, Suite 101, Tampa, FL 33625.
5. The Garnishee is authorized to deduct from the gross pay of the Defendant
the amount of $5.00 for the first deduction and $2.00 for each deduction thereafter to offset
the costs in processing the garnishment.
6. PMI shall furnish the Garnishee with a Satisfaction of Judgment in
Garnishment when the Judgment, including all costs and interest, is paid in full. PMI shall
advise the Court in writing of compliance with this paragraph.
