be made at Motion Support Office, Room 130, to be held at the Courthouse, located at 60 Centre
Street, New York, New York 10007, on the 1“ day of October, 2012, at 9:30 am or as soon thereafter
as counsel can be hear, for an order of preclusion pursuant to CPLR 3042(c ) for failure to supply
a bill of particulars, and/or for an Order striking plaintiff's complaint pursuant to CPLR 3126 for
failure to comply with defendants’ discovery demands.
The above entitled action is for personal injuries, specifically dental malpractice.
An affidavit that a good faith effort has been made to resolve the issues raised in this motion
