PLEASE TAKE NOTICE that the undersigned hereby demands that the plaintiff,
ALICIA ARUNDEL, produce for discovery and inspection with leave to photocopy, at the office
of the undersigned, AHMUTY, DEMERS & McMANUS, within twenty (20) days the
following:
1. Copies of the photographs taken on the date of the incident;
2. Copies of all receipts for co-payments and out-of-pocket expenses allegedly
incurred as a result of this incident;

3. HIPAA compliant authorization to obtain the records for the plaintiff from her
mother's health insurance company;
4. Copies of all photographs taken of the plaintiff depicting any scars and injuries
resulting from the incident;
5. HIPAA compliance authorization to obtain plaintiff's records from any gyms that
she has attended since the date of the incident;
6. HIPAA compliant authorization to obtain the plaintiff's prescription records from
allpharmacies that plaintiff has utilized subsequent to the incident;
7. HIPAA compliant authorization to obtain the plaintiff's records of Dr. Wong
(Stony Brook) and/or other dermatologist;
8. HIPAA compliant authorization to obtain the plaintiff's records of Dr. Jacobs
and/or other plastic surgeon;
9. HIPAA compliant authorizations to obtain the plaintiff's records of any treating
orthopedic doctor at Stony Brook;
10. HIPAA compliant authorization to obtain the plaintiff's records of Alex Sherry
(psychiatrist, Mather Hospital);
11. HIPAA compliant authorization to obtain the plaintiff's records of Dr. Ramasamy
and/or Dr. Liebling (Stony Brook);
12. HIPAA compliant authorization to obtain the records of Samantha Silverberg;
13. HIPAA compliant authorization to obtain the records of Dr. Galal;
14. HIPAA compliant authorization to obtain the records of the medical
provider/facility that conducted the nerve testing on the plaintiff;

15. Copies of the emails and handouts for the home exercises that plaintiff was given
by her physical and/or occupational therapists;
16. Copies of receipts for the limousine ride taken on the date of the incident and
prior limousine rides;
17. The names and contact information of any police officers, fire fighters and
responders that plaintiff has stayed in contact with since the date of the incident;
18. Contact information for Ms. Canberg.
PLEASE TAKE FURTHER NOTICE, that these authorizations are to contain the full
name and address of each provider and all other information necessary for the defendant to
obtain the requested records, including completion of Section 9(a) regarding
Alcohol/Drug/Mental/HIV treatment. These authorizations must be an original and specifically
designate and authorization AHMUTY, DEMERS & McMANUS to receive these records.
PLEASE TAKE FURTHER NOTICE, that the foregoing are continuing demands, and
that if any of the above items are obtained after the date of this Demand, they are to be furnished
prior to trial.
PLEASE TAKE FURTHER NOTICE, that upon your failure to produce the aforesaid
documents at the time and place required in this notice, a motion will be made for the appropriate
relief from the Court.
