1 My name is Paula Ashcraft, and | am the CEO for Greentree Legal, LLC, an Ohio limited liability
company (“Greentree”), engaged in providing service of process with offices at 3481 S. Dixie
Highway, Suite 263, Franklin, OH 45005.
lam over eighteen (18) years of age, a citizen of the United States, a legal resident of the State
of Ohio, and hold a valid Ohio Driver's license.
| have not been convicted of any felony, offense of violence, or offense involving dishonesty or
false statement in the last ten years, and not currently under community control sanctions,
probation, post-release control, or parole.
To my knowledge:
