I, MARY BUTLER, hereby declare:
1. I am employed as a Case Manager by Simpluris, Inc. (“Simpluris”), the Settlement
Administrator in the above-entitled action. Our Corporate Office address is 3194-C Airport Loop
Drive, Costa Mesa, CA 92626. My telephone number is (714) 640-5607. I am over twenty-one years
0f age and authorized t0 make this declaration 0n behalf 0f Simpluris and myself.
2. Simpluris is a class action Settlement Administration Company located in Costa Mesa,
California. It was founded by individuals who have each managed hundreds 0f settlements, along with
professionals in the areas 0f software development, third-party claims administration, mail-house
10 operations, and call center support management.
11 3. On August 27, 2021, Simpluris received a deposit in the amount 0f $127,150.3 1.
12 4. On September 3, 2021, Simpluris issued Settlement checks t0 the 1,472 Participating
13 Class Members. The checks were mailed Via USPS.
14 5. The following amounts were also paid from the first disbursement:
