Copies of all documents showing your income and deductions from your income including the three (3) latest income tax returns filed by you, individually and/or jointly, and pay stubs for the past six (6) months. 2. All receipts, canceled checks, or other documents showing child support payments that you have made for the past year to any household other than your own. 3. If you are claiming that you are unemployed or if you are injured, ill or disabled, bring proof of your efforts to find new employment or medical and doctor’s statements that prove you are unfit to work and medical evidence from your physician describing your condition, and stating whether or not you are disabled from working. 4. If you have applied for any type of disability payments, copies of your application and all correspondence from the government agency or any other entity to which you applied. 5. Any documentation to support any claim you may have under O.C.G.A. § 19-6-15 to show income, adjustments to income, or reasons for deviation from the child support tables, including shared custody or visitation for more than 90 days as well as: a. If you are claiming that you have other children that live with you that you are supporting, you will need to bring proof that: 1. You are legally responsible for the child. 2. The child is in fact in your home. 3. You are providing support to that child. b. Any documents that show the cost of any health or other insurance that you are providing for the child. c.
