The Plaintiff, Joseph A. Ortiz, Individually and as Administrator of the Estate of Jospeh
M. Ortiz, having duly moved this Court, Ex Parte, for an Order seeking substituted service of
process upon.the defendant, John Frew, upon his insurance companies, Progressive Insurance
Company, and Allstate Insurance Company and further seeking service on defendant Anh Ly,
by directing substituted service of process upon his insurance company Allstate Insurance
Company, and having sought this relief by way of a Ex Parte Notice of Motion, dated November
1, 2023, and the affidavit of C..Daniel McGillicuddy, Esq., attorney for the Plaintiff, sworn to
November 1, 2023, with attached Exhibits, and upon reading and filing of the above papers, and
due deliberation having been had, it is;
ORDERED, that the Ex Parte Motion of the Plaintiff as it pertains to defendant John
Frew, is granted in it's entirety, and the substituted service of process upon defendant John Frew
be had by service by mailing a copy of the Summons and Complaint and this Order to

copy of the Summons and Complaint and this Order to Allstate Insurance Company at 1125
RXR Plaza, Uniondale, New York 11556, and it is further;
ORDERED, that the Ex Parte Motion of the Plaintiff, as it pertains to defendant Anh Ly,
is granted in it's entirety, and that substituted service of process be had upon defendant Anh Ly
by way of service of a Summons -and Complaint and this.Order upon his insurance company, by
mailing a copy of the Summons and Complaint and this Order to Allstate Insurance Company
at 1125 RXR Plaza, Uniondale, New York 115563 .
ORDERED, that service of the afore mentioned defendants is extended to December 30,
2023. .
