REASON POR ADNXSSION: Right ankle injury.
HISTORY OF PRESENT ILLNESS: This is a 71-year-old female with
past inedical history of AFib, on Bliquis, recent ischemic stroke,
who presented to the Emergency Department from Elderwood Nursing
Home after sustaining a right ankle fracture. She was in
Elderwood for rehab after having a stroke with residual
right-sided weakness and was well
doing until yesterday when she
was standing and nursing assistant was trying to place her diaper
on her when she tripped and .Eell and schsequently broke her
ankle. X-rays were obtained at that time, and the patient was
found to have a himalleolar ankle fracture.. Dr. Szczech from
Oethopadics was called at that time and the patient was admitted
for planned surgical intervention this morning. Dr. smosech came
and saw the patient this and given the fact that the
morning
patient's right ankle was extremely edematops, inflamed, he
thought it was best to delay surgery up until next week. Due to
the fact that the patient is hemodynamically stable it is thought
best that the patient he returned to Elderwood for the week until
the
day before her planned surgical intervention which will be
Nonday, 11/30.
DISCRARGil INSTRUCTIONS: She will he discharged back to Elderwood
and should return on Sunday night which is 11/23 for preap
interrogation for planned surgical interventinn on Honday.
DISCEARGB NEDICATION8: The patient should take Lovenox up until
Sunday moraing and should also eake Ultram 50 mg p.o. q.4 for
pain. sha should also continue her home meds except bold her
Eliquis at this time.
