I, AUSTIN T. DAILEY, ("Affiant"), an attorney in the law firm of KLEIN & KLEIN,
LLC, being sworn, certify that the following information is true:
1 I have retained a duly licensed private investigator to make diligent search and
inquiry to confirm or discover the name, residence and age of the Defendant VICKI DELYS
HYDE, and to effect service of process upon such Defendant. The results of such diligent search
and inquiry are as hereinafter set forth.
2 After making a diligent inquiry, Affiant has been able to confirm that VICKI
DELYS HYDE is over 21 years of age.
3 After making a diligent inquiry, the last residence address Affiant has been able
to confirm for VICKI DELYS HYDE is 639 NE 18th Ave., Ocala, FL 34470.
4 After making diligent inquiry and effort, Affiant has been unable to effect service
of process upon VICKI DELYS HYDE, as such Defendants have concealed themselves to that
process cannot be served.
5 Affiant knows of no one and believes there is no person on whom service of
process would bind Defendant VICK] DELYS HYDE.
6 Affiant understands that he is swearing or affirming under oath to the truthfulness
of the claims made in this Affidavit and that the punishment for knowingly making a false
statement includes fines and/or imprisonment.
