I, Judy L. Schmidt, MD, FACP, declare and state, that if called upon to testify and sworn, I
could and would testify to the following facts of my own personal knowledge.
Qualifications and Credentials
10 1. 1 am atriple board certified Internist, Hematologist and Medical Oncologist. My three year
1] subspecialty fellowship training in Hematology and Medical Oncology was completed at the Mayo
12 Clinic in Rochester, Minnesota in 1988. Patients traveled from all over the world to obtain second
13 opinions and medical care from the Mayo Clinic. As a Fellow in Hematology and Medical Oncol-
14 ogy, I was exposed to a plethora of patients with thrombophilia and many patients with myeloprolif-
15 erative neoplasms. I worked along side world experts in myeloproliferative neoplasms and throm-
16 bophilia.
17 2. In 1988, I moved to Missoula, Montana and practiced community-based private practice
18 Hematology and Medical Oncology for 22 years. Eighty percent of Hematology/Oncology is prac-
19 ticed by community-based physicians. I was a clinical instructor for the University of Washington
20 WAMI (Washington/Alaska/Montana/Idaho) program for several years teaching the University Med-
21 ical Students Hematology and Oncology. In my solo Montana practice, I was responsible for the in-
22 ternal medicine care of over 95% of the patients I cared for. I was involved in the diagnosis and
23 treatment of hundreds and hundreds of thrombophilia patients; including patients with myeloprolif-
24 erative neoplasms.
