5. You will be expected to lead a reasonable and stable lifestyle; work at suitable ernployment or pursue study or training
leading to employment; support your dependents and meet other family responsibilities.
6. Refrain from frequenting unlawful or disreputable places or consorting with disreputable persons.
7. A copy of your pre-sentence report will be released to the appropriate treatment agency and you will be required to
sign release of information forms allowing the Probation Department and the Court access to your treatment records.
8. You shall submit at any time, without notice and without a search warrant/order to a search of your person, residence,
and other personal property, leased and/or owned that is not your residence and any area under your
vehicle,
immediate control and permit the seizure of any narcotic implements, illegal drugs, gambling records, weapons, stolen
and/or or contraband deemed to be detrimental to the rehabilitative goal of the probationer's

9. At the discretion of the Office of Probation, you will faithfully attend and successfully complete a Moral Reconation
Therapy (MRT) Program and/or a Cognitive.Behavibral Therapy (CBT) Program.
10. You will submit to the appropriate DNA testing by providing a DNA sample suitable for testing for the Convicted
Offender Index, as mandated by Executive Law §995-c for designated offenders OR provide a DNA sample suitable for
testing for the Subject Index as authorized by Title 9 NYCRR part 6192. The aforementioned requirement does not
apply to those serving a period of Interim Supervision or those placed on probation supervision subsequent to a Youthful
Offender Adjudication.
11. You will perform __50__ __ hours of community service within __6 months of sentencing for a public or not-for-
profit corporation, association, institution or agency.
12. You shall not consume or possess alcohol, illegal drugs, drugs not prescribed to you or synthetic substances that
duplicate the effect of illegal or prescription drugs, drug paraphernalia and shall submit to recognized tests to determine
their use. Do not use or possess any product designed to alter the results of recognized drug tests. Immediately advise
your supervising probation officer of any medications you are taking.
13. At the discretion of the Probation Department you will complete a mental health, and/or alcohol/substance abuse
evaluation. This evaluation must be completed by an OASAS licensed Alcohol/Substance Abuse Center. . You will
successfully complete any recommended treatment, in-patient, out-patient and/or residential, and follow any other
treatment recommendations, which may include participation in a sobriety support group. A copy of your pre-sentence
report will be released to the appropriate treatment agency, and you will be required to sign release of information forms
allowing the Probation Department and the Court access to your treatment records. You may be responsible for any
costs incurred for said treatment
14. You will not enter or remain in bars, taverns, nightclubs, liquor stores or purchase/possess any alcoholic beverages.
15. You will sign a release of information allowing the Probation Department to receive information about your medical
treatment, and to share information about your probation case with your health care provider(s).
16. You will participate in an Anger Management program and will be responsible for any costs incurred as a result of
said program.
17. You will observe the specified conditions of conduct as set forth in an Order of Protection issued by this or any other
Court.

memorandum to the court summarizing the offender's adjustment to interim supervision, and where
appropriate, to make a recommendation on disposition and/or sentencing, the following is submitted:
The defendant has been reporting to this department for community supervision, as directed, since January
2016. The defendant has remained arrest-free during this time period. He, however, was issued a ticket for a
traffic violation -VTL 1151A - Failure to Yield right of Way to Pedestrian/Crosswalk, on 6/27/16. On 7/26/16,
the defendant paid a fine and the matter was disposed of in Wappingers Falls Village Court. The defendant
has been randomly drug-tested on multiple occasions while being supervised and all tests have been negative.
The defendant has been working with a case plan which stipulates that he conducts a law-abiding lifestyle;
Anger Management training as well as completing 50 hours of Community Service. He attended
completing
Community Transition Center where he successfully completing Anger Management training as well as the 50
hours of Community Service. During this time the defendant continued attending Dutchess Community College
where he studies Criminal Justice. The defendant has also maintained his employment at The ARC of
Dutchess.
CPL 390.30 (6-b) states that if a defendant has satisfactorily completed a term of interim supervision, he or she
shall receive credit for time served under the period of interim supervision towards any probation case
subsequently imposed in that case.
If the defendant is sentenced to a term of probation, please advise this department. Otherwise we will
be closing interest in this case as of the maximum expiration date.
