2. This is a claim for personal injuries by my mother,SHIRLEY
PETWAY, all as a result of thenegligence of the respondent hits ownership,
. operation,malutenance, supervision,managementand control of the
premises focated at 70 Malcolm X Soulevard, New York, NY owned and
opersled by the respondent. .
& On August 24, 2019, while visiting my inother, SH)RLEY

necessary to tile a Notice of Claim and since my mother's acoldent she had
lea hip surgmy and been sinos.
6. My mother sustained sedous pomonalinjuries including but not
liniRed to a fractured hip requiringino (2) surgeries to date as a result of her
faBdue to the negggence of aspondent.
7. No priorappilcallon for the relistsought herein has hegeloline been
made.
