Defendant requested a credit account ("Account") be opened, and an Account was opened on or about 02/08/2013 for the
Defendant's use. The previous or original creditor is CREDIT ONE BANK, NA. The account was sold. Plaintiff acquired
and is now the owner ofthe account which is the subject matter of this action. The original Account number (now closed) is
XXXXXXXXXXXX8OS7.
Defendant is indebted to Plaintiff on the overdue unpaid balance owing on the Account of $1,433.80 ("Account Balance").
Defendant failed to pay the Account Balance. Defendant is indebted to Plaintiff in the amount of $1,433.80, on an account
stated on or about 01/14/2016. The last payment was made on or about 06/10/2015.
As a direct result ofthe Defendant's failure to pay, Plaintiff is damaged in the Account Balance amount
2. Ibelieve the Defendant is at least 18 years old.
3. I believe the Defendant is not in the military service.
