BEFORE ME, the undersigned authority, personally appeared REHAN N. KHAWAJA,
Attorney for Plaintiff, who being first duly sworn, deposes and says as follows:
1 He is the Attorney for Plaintiffs.in this action.
2. He has made diligent search and inquiry to discover the residence of the
Defendants JACK JOHN WILLIAMS and GERTRUDE R. WILLIAMS and the sameis set
forth in this sworn statement as particularly as is known to the Attorney for Plaintiff. That he has
checked the telephone directory to find the above-named Defendants’ telephone number and
address, and that said name is not listed in the directory.

3 He has searched for the above-named Defendants’ names in the probate files in
Duval County, and no such name appears in the records except for as provided below in
paragraph 8.
4 The above-named Defendants are over age of 18.
5 The last known address for the Defendants Jack John Williams and Gertrude R.
Williams is 2129 Yulee Street, Jacksonville, Florida 32209.
6 The present residences of the above-named Defendants are unknown, and there is
no person in the state upon whom service of process would bind said absent Defendant.

That the undersigned searched for the Defendants by searching the data base
provided by Lexis Nexis legal research and based on the results, the Defendants are deceased.
8 That there was a probate proceeding in the name of Gertrude Williams in Duval
County, Florida, under case number 2019-CP-2946, however, undersigned can not determine if it
relates to the same person as Defendant herein.
