 On the 11th day of September, 2022, this Court placed the above named Defendant on probation for a period
 of 6 Months for the offense(s) of Battery. The Order of Probation required that the Defendant comply with
 certain general and specific conditions of probation. The Jacksonville Sheriff's Office Misdemeanor Program
 has filed a sworn "Memorandum Alleging Violation of Probation" (attached), from which it appears that the
 Defendant has violated the probation previously imposed in this cause in a material respect. Based upon the
 sworn allegations presented to the Court, it is hereby:


 CONSIDERED AND ORDERED:


 That the said Davis, Deangelo is hereby ordered to show cause before this Court as to why the Order of
 Probation in this cause should not be revoked, set aside and vacated, and the Defendant be sentenced as
 provided by law, based upon the allegations contained in the "Memorandum Alleging Violation of Probation"
 (attached), which is hereby incorporated by reference."

         The Defendant shall appear before this Court on                            20,       at          a.m./p.m.
         in Courtroom Number,           , to answer allegations that this probation has been violated. The Clerk is
         directed to calendar this cause for that date, and to issue a summons directing the Defendant to appear (a
         copy of this Rule shall be attached to the summons).

We believe that the above named Probationer has violated the following conditions of the probation order in this
cause which states:
    .   You will make a full and truthful report to your counselor in person, a minimum of once per month or as
        otherwise directed by your counselor. You will answer your counselors questions truthfully.

           You will contribute $60.00 per month supervision fee beginning on a date set by your probation officer.
           (948.09 FS).
           You will enroll in and complete an Anger Management course.

           You will pay a fine/cost totaling $655.00.

           You will perform 25 hours of community service.



Said Probationer has allegedly violated the above condition(s) in that he/she:
      .    Failed to report on 12/27/2022

           Has paid $12.00 in probation fees. As of 02/07/2023, is in arrears $348.00

           Failed to show proof of enrolling in and/or completing an Anger Management course.

           Failed to show proof of paying $655.00 fine/cost in full. As of 02/07/2023, receipt balance is $655.00.

           Failed to show proof of performing 25 hours of community service.
