knowledge and are true and correct.

"The child's present address is 198 Springbank Drive #7, London, Ontario, Canada
N6J1G1.
"For the past five years immediately preceding the date of this affidavit, the child has lived
at the following addresses with the following persons:

Persons lived with: _VASHTI SAVANNAH WELLS
Dates: 2017 - Present
Address: 198 Springbank Drive #7, London, Ontario, Canada N6J1G1
