written admission of the following relevant matters of facts.

4. Admit that you have never held shares in Evergreen Adult Day Care in Flushing,
Inc.
5. Admit that you have never held shares in Evergreen J.S.C. Adult Day Care in NY,
Inc.
6. Admit that you have never held a membership interest in Evergreen Flushing New
York LLC.
7. Admit that you have never held shares in Evergreen Homecare Service of NY,
Inc.
8. Admit that you did not pay any monetary consideration for your alleged interest
in Elim Home Care Agency, LLC.
9. Admit that you were never approved by the New York Department of Health to
be an owner or profit participant in Elim Home Care Agency, LLC or in
Evergreen Homecare Service of NY, Inc.
10. Admit that you did not pay any monetary consideration for your alleged shares of
Evergreen Homecare Service of NY, Inc.
11. Admit that you never had a written employment contract with any of the
Defendants. .
12. Admit that you received more than double your investment in a share of the
profits of SR Homecare of NY, Inc. prior to itsclosure.
13. Admit that you were present at numerous meetings in which the sale and
dissolution of SR Homecare of NY, Inc. was discussed.
