The Bureau of Home Care Licensure and Certification,Division of Home and Community Based
Services and the Bureau of Health FacilityPlanning and Development, Division of Legal Affairs have
reviewed the proposed Management Agreement between Elim Home Care Agency, LLC (License #
2344L001) and Daniel Park (Manager) and find itacceptable. A photocopy of the approved
management agreement isenclosed.
Please be advised that the approval of a management agreement does not autherize the manager to
directly employ or cóñtract with individual nurses, home health aides or personal care aides to
provide services to the current operator's patients. Therefore, Elim Home Care Agency, LLC must
continue to employ or have individual contracts with the direct care staffprovidiñg care to these
patients.
