1. I am a Plaintiff in the action herein. I have read the Summons and Verified Complaint
and know the contents thereof, and the same are true to my knowledge, except those
matters therein which are stated to be alleged upon information and belief, and as to those
matters I believe them to be true.
2. On January 11, 2018, I was injured while working at a building under
construction/renovation at 2357 60th Street, Brooklyn, New York 11204.
3. As a result of this accident, I suffered multiple serious injuries.
4. I have been advised by my attorneys that I have a meritorious claim under New York
State's Labor Law.
