true and correct copy of my Judgment of Marital Dissolution, entered on June 3, 1974, dissolving
my marriage to decedent. Although we never remarried, we subsequently reconciled and lived
together the last 14 years prior to his death.

I declare under penalty of perjury under the laws of the State of California that the
