CABOT COACH BUILDERS, INC., intends to call Dr. Suzanne L. Tuzel, M.D., to testify as an
expert witness at the time of trial in the above-captioned matter. Dr. Tuzel is a Diplomate American
Board of Psychiatry & Neurology and a licensed medical practitioner in the State of New York. Dr.
Tuzel will testify based upon, but not limited to, a review of the plaintiff, Melissa A. Crai's medical
records, and examinations of the plaintiff.
A copy of Dr. Tuzel's report and curriculum vitae are annexed hereto.

this examining physician's office, North Shore Psychiatric
Consultants, 222 Middle Country Road, Suite 210, Smithtown, New
York, on February 25, 2020. Ms. Crai arrived 15 minutes late for
her appointment scheduled at 4:00 p.m. A thorough evaluation in
the form of her New York State Driver's License was reviewed. It
was explained to the examinee that this appointment was for the
purpose of evaluation only and therefore no doctor-patient
relationship would be established.
