1. I am a Vice President-Document Execution of.TPMorgan Chase Bank, National
Association ("Chase"), and Iam authorized to make thisaffidavit on behalf of Chase. Chase is
the loan servicer and the Plaintiffin thisaction.
2. I make this affidavitbased on my review of Chase's business records, as more fully
described below in paragraph 4. Iam over the age of 18 and competent to testifyas to the matters
contained in this affidavit.
3. A note dated 11/13/2006. in the original principal amount of S 258.000.00 (the "Note")
was signed by or on behalf of WALKER. CELIA A ("Borrower"), and was secured by a
mortgage on a property located at 151 BEACH 61ST ST ARVERNE. NY 11692 (the
"Mortgage"). The Note and Mortgage, and ifshown and selected below, any Loan Modification
Documents."
Agreements are collectively referred to as the "Loan
X The loan was modified by a Consolidation Extension Modification Agreement
("CEMA"), combining certainprevious Notes and Mortgages described in Exhibit A to
the CEMA.
X The loan was modified by a Loan Modification Agreement (loss mitigation) with a
firstmodified payment due on 12/01/2018.
4. As a mortgage servicer, Chase collects payments from Borrower and maintains
up-to-date electronic records concerning the loans itservices in itselec=nic record-keeping
system. I have access to the business records of Chase, including the business records forand
relating to the Borrower's loan. I make thisaffidavit bued upon my review of those records
relating to the loan and from my own personal knowledge of how they are kept and maintained.
The loan records are maintained by Chase in the course of itsregularly conducted business
activities and aremade at or near the time of the event, by or fmm information transmitted by a
person with knowledge. Itis the regular practiceto keep such records inthe ordinary course of a
regularly conducted business activity.Chase's records thatrelate to the loan that Ireviewed and
relied upon for the statements made in this Affidavit include images of the Note, Chase's
clectronic servicing system, and images of correspondence to the borrower.
5. Plaintiff,directly or through itsagent, is inpossession of the originalNote and was in
possession at the time of filingthe Complaint, a true and cormct copy of the Note isattached to
the Affidavit.
6. Borrower failed to make the payment that was due for 8/1/2019 under the Loan
Documents and has failed to make subsequent payments to bring the loan current, and the entire
loan balance isnow due and owing toPlaintiff. A copy of the Chase business record showing
amount due and the delinquency date is attached as Exhibit C.
7. As of 10/11/2021 (unless specified otherwise below), Plaintiff seeks to recover the
following itemized sums of money that are due and owing under the Loan Documents, exclusive
of attomey's fees and costs in this action:

9. My aview of records maintained by Chase reveals the following regarding the sending of
