Personally appeared, JENNIFER M. JASON o¢ the City of Hartford, County of Hartford and State of
Connecticut, who, being duly sworn, does make affidavit and say:
1. That he/she is over eighteen years of age and believes in the obligations of an oath;
2. That he/she is an Attorney employed by Hunt Leibert Jacobson, P.C., counsel for the Plaintiff in the
above-captioned matter;
3. That this affidavit relates to Plaintiff's compliance with the terms of the Emergency Mortgage
Assistance Program, C.G.S. sections 8-265aa et seq., as amended by P.A. 12-1 (the "Act").
4. That the plaintiff is a "mortgagee" as defined in Section 8-265ce of the Act;
5. That the plaintiff has fully complied with the requirements of subsection (a) of C.G.S. section
8-265ee of the Act by mailing to MICHAEL P COLANGELO and DAWN L COLANGELO, the
mortgagor(s) on March 22, 2013, at 32 Falcon Ridge Road, Torrington, CT 06790 by registered or certified
mail a notice (the "Notice") containing all of the information required by that subsection. A copy of the
Notice is affixed hereto, marked Exhibit "A";

currenily accepting applications for emergency assistance payments under the EMAP Program.
You have 60 days frorn the date of this notice to:

to the Authority witn Bank of America, N.A. or a face-to-face meeting with one of the
Authority’s designated consumer credit counseling agencies (see attached list of
agencies) to attempt to reach an agreement to resolve the delinquency or default by
restructuring the loan payment schedule or otherwise; AND
Contact the Authority at the address and phone number contained in this notice to obtain
information and apply for Emergency Mortgage Assistance payments if you are unable to
resalve the mortgage delinquency or default.

Under EMAP, foreciosure may not be commenced before this notice is mailed to you.
Also, a foreclosure judgment cannot be entered during the period which begins the day
you become eligible to apply for emergency mortgage assistance and ends when (i) you
and the mortgagee reach an agreement to resolve the delinquency or default; or (ii) the
Authority reaches a determination regarding your application for emergency mortgage
assistance, or thirty calendar days after the Authority receives your application,
whichever is sooner.
If you do not meet or have a telephone conference with us or with a designated
consumer credit counseling agency as set out above, or if you do not comply with the
terms of any agreement to resolve the delinquency or default, or if you do not contact the
Authority and apply for emergency mortgage assistance within thirty (30) days of any
default in payment under an agreement to resolve the delinquency or default, (1) you will
not be eligible to apply for, or to receive, emergency mortgage assistance payments, and
(i) foreclosure of your mortgage may continue without further restriction or requirement.
An application for emergency mortgage assistance does not preciude you from applying
for mediation through the court.
