Dear Andrew,
At my raquest, a meeting was held with you and other agency representatives today, to
review several patisñt care reports, as I had significant concerns over the care that you
provided to patients. Within these reports, numerous protocol violations were also
identified.
One of the roles of the Agency Medical Director is to protect the patients we are
responsible for caring for. When the Agency Medical Director has signi1icant concems
over a provider's ability to competently provide medical care, untilthose concerns can
be completely resolved, itmay be necessary to restrictthe practice of the provider
within the agency. At this time, I am notifying you that I am placing you under such a
restriction at Wayne County ALS.
Based on my review of the patient care reports that were discussed today, and other
quality issues identified by the quality assurance program at the agency, I am restricting
you from any contact with patients. In oder to lift
this restriction,I am requiring you to
recertify your paramedic certification through a fullrefresher class (no online or CME
component), followed by satisfactory completion of a paramedic practical and written
recertification exam, and fullre-clearing at the regional and agency level. l. have come
to this decision based on the identification of protocol violations and incorrect patient
management over a wide range of patient types and presentations. I am unable to offer
such an extensive remediation plan in-house.
