CAROLINE K. HINSHAW, declares:
1. lam an active member of the California State Bar and a shareholder in BRYAN =
HINSHAW, a Professional Corporation. | am Court appointed guardian ad litem for NATALIE
FERRERO and NICHOLAS FERRERO, beneficiaries of the Revocable Living Trust Agreement
of CHARLES ACTIS dated November 2, 2004.
2. l rendered services during the period from September 1, 2005 through May 22,
2007, inclusive on behalf of NATALIE FERRERO and NICHOLAS FERRERO, the Trust
beneficiaries. Such services are briefly summarized as follows:

3. 1 expended a total of 10.75 hours rendering such services for the benefit of both
Trust beneficiaries, NATALIE FERRERO and NICHOLAS FERRERO.
4. The undersigned alleges that said professional legal services were necessary
and that the charges therefor were reasonable in the circumstances. Attached hereto, marked
“Exhibit A” and incorporated by this reference is a statement of all services rendered for both
NATALIE FERRERO and NICHOLAS FERRERO.
5. l received assistance from paralegals of Bryan -Hinshaw in rendering such
services for the benefit of both Trust beneficiaries, NATALIE FERRERO and NICHOLAS
FERRERO. The total expended hours rendering such services by both myself and the
paralegals are 73.71 and the total legal fees are $9,052.25. Notwithstanding the foregoing, as

a courtesy to the minors, | am billing only for my time. | therefore respectfully request an Order
of the Court authorizing DEBRA J. DOLCH, Successor Trustee of the Revocable Living Trust
Agreement of CHARLES ACTIS dated November 2, 2004 to pay compensation to BRYAN «
HINSHAW the total amount of $2,769.75.
WHEREFORE, the undersigned respectfully requests Orders of this Court:
1. Authorizing and directing the Trustee of the Revocable Living Trust Agreement off
CHARLES ACTIS dated November 2, 2004 to pay the sum of $2,769.75 as reasonable
compensation for services as specified to BRYAN « HINSHAW or such other amounts as the
Court may determine;
2. For all other and further Orders as to the Court may seem necessary or proper.
