applicable statutory fees, directing Respondent to
(1) create or issue, produce, and deliver to Petitioner two (2) long-form
certified copies of (a) the New York State birth record, signed with seal affixed, for Petitioner's
father, Fiore Massaro, who is one and the same as Fiore D. Massaro, declaring that he is
One and the Same and aka Fiorentino Massaro, Fiore Massaro, Fiore D. Massaro, Fiore
Dominic Massaro, Fiore Dominick Massaro, Frank Massaro and Frank Massaro, Sr.
born in White Plains, New York, on September 19, 1925, which record is necessary for the
legal, proper and sole purpose of enabling Petitioner to apply for and obtain recognition of dual
citizenship of the Republic of Italy.
(2) create or issue, produce, and deliver to Petitioner two certified copies of
Order declaring that Petitioner's grandfather is One and the Same as Dominick Massaro
and Dominic Massaro.
(3) create or issue, produce, and deliver to Petitioner two certified copies of
Order declaring that Petitioner's grandmother is One and the Same as Maria Della
Peruta, Maria Peruto, Maria Pruta, and Mary Massaro.
(4) create or issue, produce, and deliver to Petitioner two certified copies of
Order declaring that Petitioner's mother Milagro Ventura Massaro, is One and the Same
as Milagro Ventura Llorens and Milagro Ventura Y Llorens, as proven by Exhibits J
and L.
create or issue, produce, and deliver to Petitioner two certified copies of
