COMES NOW Chase Orion Godfrey, the Petitioner, pro se, and shows to the Court:
1. The full and correct name of Petitioner is Chase Orion Godfrey.
2. Petitioner is now and has been for more than thirty (30) days next preceding
the filing of this Petition, domiciled in the State of Oklahoma and has been a resident for
more than thirty (30) days preceding the filing of this Petition an actual resident of
2 Oklahoma County.
| 3. The Petitioner was born on the 28" day of November, 1988, in Oklahoma City,
Oklahoma County, State of Oklahoma.
4. The birth certificate number of the Petitioner is 135-88-039487 and the birth
is registered in the State of Oklahoma.
5. Petitioner desires to have his present name changed from Chase Orion
Godfrey to “Rhea Celeste Danann,” because the Petitioner is transgender, has used this
name for an extended period of time, and feels it fits the Petitioner's gender identity and
expression.

changed from male to female, and has attached a letter from his current physician in
support of said request. (See Exhibit “A”)
7. Petitioner requests that the Court issue an order that a new birth certificate
be issued reflecting the gender and name changes sought by this Petition.
8. Petitioner does not seek to have his name changed for any illegal or
fraudulent purpose, or to delay or hinder creditors.
WHEREFORE, your Petitioner prays for an Order and Judgment of this Court changing
his name from that of Chase Orion Godfrey to Rhea Celeste Danann, changing his gender
from male to female, and for such other and further relief as may be appropriate.
