ORDER FOR CHANGE OF NAME
THIS MATTER came on for hearing this 13" day of March, 2021, before the

Honorable Richard Ogden, Judge of the District Court in and for Oklahoma County, State of
Oklahoma, upon the verified petition of Kristen Leigh Rayos, on behalf of the minor child
OLIVIA GRACE RAYOS for change of name, filed in this Court on the 27" day of April,
2021: and the Petitioner appeared personally and by her attorney, Christine S. Schem, and
the Court having heard evidence of the witness duly sworn, finds:

1. The Petitioner and the minor child are now and have been for more than 30
days next preceding the filing of the Petition domiciled in the State of Oklahoma and have
been actual residents of Oklahoma County for more than 30 days preceding the filing of the
Petition.

2. Petitioner attempted to give notice of the filing of said petition to the natural
father of the minor child via personal service by process server at the natural father's last
known address in Springtown, Texas. As evidenced by the Affidavit in Support of Substitute
Service contained by the files of this court in the above styled and numbered cause, the
process server was unable to personally serve the natural father. The natural father has not
seen or spoken to the minor child in more than two years.

3. Notice of filing of said Petition has been given by publication in a newspaper
of general circulation in Oklahoma County at least ten days prior to this hearing as more

particularly reflected by the proof of publication contained by the files of this court in the

Order for Change of Name - OGR Page 1 of 3above styled and numbered cause. Petitioner is not required to register as a sex offender
pursuant to the Oklahoma Sex Offenders Registration Act.

4. The minor child, Olivia Grace Rayos, was born on October 9, 2008 in
Oklahoma County, State of Oklahoma, United States of America.

5. The birth of minor child, Olivia Grace Rayos, is registered with the State of
Oklahoma, Oklahoma Vital Records and Health Statistics Division; State File No. 135-
2008-044499.

6. The Petitioner, on behalf of the minor child, desires to have her minor
daughter's present name changed from OLIVIA GRACE RAYOS to OLIVIA GRACE
DOWNING, that said Petitioner, on behalf of the minor child, is entitled to such relief for the
reasons set forth in said verified Petition and that Petitioner does not seek to have the minor
child's name changed for any illegal or fraudulent purpose or to delay or hinder creditors.

IT IS THEREFORE ORDERED, ADJUDGED AND DECREED by the Court that
Petitioner's minor daughter's name is hereby changed from OLIVIA GRACE RAYOS to
OLIVIA GRACE DOWNING and that copies of this order may be furnished to the necessary
agencies to make such changes upon Petitioner's official records with such agencies.
