THIS MATTER was heard by this Court on November 14, 2018, and having heardTargument of
counsel, review ofthe court file, and being otherwise fully advised in the premises, it is—
ADJUDGED as follows:
1. All parties were properly noticed and the only person in attendance was Jonathan I.
Rotstein as counsel for the Plaintiff.
2. The Court had received copies of the two Optum liens showing that Optum had paid
through Medicaid the balance owed to Coastal Neurology.
3. The Coastal Neurology Letter of Protection is disallowed as it would be balanced billing,
which is prohibited under Medicaid and Medicare laws.
4. The Optum lien in the amount of $823.99 is reduced to $494.39.
5. The Optum lien in the amount of $2,774.99 is reduced to $1,664.99.
6. The attorney's fee is $4,000.00, with costs in the amount of $616.50, for a total of
$4,616.50.
7. The Plaintiff shall receive the net sum of$3,224.12.
