PLACEMENT ORDER
On June 3, 2021, a Status Hearing was held pursuant to Subchapter C, Chapter 263, Texas Family
Code. The Court entered in the following placement orders at the conclusion of the status hearing:

1.1. The Department of Family and Protective Services (“the Department”) appeared
through TAMMELA CHERRY-CROW, caseworker, and by attorney, JESSICA
WORTHAM and announced ready.
1.2. Respondent Mother, ASHLEAGH WILCOX appeared in person and through
attorney of record ALICIA BARKLEY and announced ready.
1.3. Respondent Presumed Father, JAMIE PHELPS, father of the child HAYDEN
WILCOX appeared in person and through attorney of record SCOTT ELLIS and
announced ready.
1.4. DONALD DAVIDSON, appointed by the Court as Attorney Ad Litem of the child
the subject of this suit, appeared and announced ready.
1.5. CASA, appointed by the Court as Guardian Ad Litem of the child the subject of
this suit, appeared and announced ready.

2. IT IS ORDERED that CAROL DYESS is authorized to consent to medical care for
the subject child, pursuant to § 266.004(b)(1), Texas Family Code. This
authorization is limited to the provision of medical care services provided by the
Medicaid program. At the request of the Department, the named individual shall

provide to the Department a written summary of the medical care provided to the
subject child since the last hearing.
2.1. IT IS ORDERED that all previous orders issued by this Court shall continue in
full force and effect subject to the following modifications:
Court ORDERS that the child be placed with CAROL DYEss until further Cohtt
orders. The Court nds this family placement to be the least restrictive placement
and in the child’s best interest.
