Nature of Proceedings: Petition for Appointment of General Guardian
The following must be submitted before the matter may be recommended for approval:
1.) An Indian Child Inquiry Attachment (Judicial Council form ICWA-010A) noting the inquiry has been made and identifying the individuals questioned, which should include someone from the paternal side of the minor's family;
2.) Proof of service showing the minor has been personally served with the Notice of Hearing and a copy of the petition at least 15 days before the hearing. [Prob. Code, § 1511
(b).] In the alternative, Petitioner may submit a Consent to Appointment (Judicial Council form GC-211) signed by the minor, which waives notice;
3.) Proof of service showing the minors’ parents have been personally served with the Notice of Hearing and a copy of the petition at least 15 days before the hearing. [Prob. Code, § 1511(b).] ; and
4.) New proposed order and letters reflecting the changes made in the amended pet
