BEFORE ME, the undersigned authority, this day personally appeared David Cramer, Esquire, who, after
being duly sworn, upon his oath, deposes and says:
1. That my name is David Cramer, and that I am a licensed attorney practicing in Orange County, Florida.
2. That I am one of the attorneys representing Plaintiff in this action against Defendant The Estate of Sallylou
Wojciechowski, Any and all Unknown Parties who claim an interest as spouses, heirs, devisees, grantees,
assignees, lienors, creditors, trustees, personal representatives, administrators or as other claimants, by,
through, under or against Sallylou Wojciechowski, deceased, Damon Wojciechowski, as Heir and Dane
Wojciechowski, as Heir.
3. As of the date of this Affidavit, the total amount of attorneys' fees which has been and will be incurred by
Plaintiff is a flat fee of $500.00.
4. Further, the following costs and expenses were incurred by Manley Deas Kochalski LLC on behalf of the
Plaintiff:
