COMES NOW, the Plaintiff, COLLINS ASSET GROUP, LLC, by and through its
undersigned counsel, and states as follows:
1 On February 14, 2018, a Final Default Judgment was entered herein against the
Defendant, HUMBERTO GONZALEZ, in the amount of Forty Thousand, Two Hundred thirty Eight
Dollars 84/100 ($40,238.84). A copy of the Final Judgment is attached hereto as Exhibit “A”.
2 Movant does not believe that said Defendant has in there possession visible property
upon which a levy can be made sufficient to satisfy the Judgment herein. Plaintiff suggests that
WELLS FARGO, located at 1710 S. Andrews Avenue, Fort Lauderdale, FL 33316 is indebted to,
or has tangible property in its possession or control of the Defendant, HUMBERTO GONZALEZ
and moves that the Clerk of the above Court issue a Writ of Garnishment in the sum of Forty
Thousand, Two Hundred thirty Eight Dollars 84/100 ($40,238.84) commanding the Garnishee,
WELLS FARGO located at 1710 S. Andrews Avenue, Fort Lauderdale, FL 33316 , named above
to answer according to law in such cases.

WHEREFORE, COLLINS ASSET GROUP, LLC respectfully requests the Honorable Court
to issue a Writ of Garnishment, and suggests WELLS FARGO, as Garnishee and issue any other
relief the court deems proper and just.

THIS CAUSE having come before this Court on Plaintiffs Motion for Default Final
Judgment, and the Court having reviewed the motion and file, and being otherwise fully advised
in its premises.
IT IS HEREBY ORDERED AND ADJUDGED:
The Plaintiff, COLLINS ASSET GROUP, LLC. by and through its undersigned counsel at
Behar, Gutt & Glazer, whose mailing address is 1855 Griffin Road. DCOTA #A-350. Fort
Lauderdale, FL 33004, shall hereby recover from Defendant(s), HUMBERTO GONZALEZ whose
mailing address is 2401 N END ST ORLANDO FL 32837 the principal amount of $38,853.45, court
costs of $510.39, and attorney's fees of $875.00. for a total judgment in the amount of $40,238.84,
for which let execution issue.
This Judgment shall bear the interest rate as allowed and adjusted by law from the date of
entry until satisfied.
IT IS FURTHER ADJUDGED that the court retain jurisdiction of this action to enter
further orders as are proper after entry of the judgment, and the judgment debtor(s) shall complete
under oath Florida Rule of Civil Procedure Form 1.977 (Fact Information Sheet), including all
required attachments and the spouse related portion, and serve it on the judgment
