A statement of the nature and substance of the Plaintiff's Cause of Action is as follows:
1. Plaintiff,a corporation, has itsprincipal place of business in Syracuse, County of Onondaga, NY.
2. Defendant(s) ANDREA HELLINGER residing at 5768 Waters Road , Lowville, New York 13367, County of Lewis.
3. Action for $3297.23, with interest from March 28, 2017 being the balance due for the agreed price and reasonable
value of medical services furnished by the Plaintiff to the Defendant and/or a dependant of the Defendant on or about
and prior to March 28, 2017; no part of said balance has been paid although same has been duly demanded by the
Plaintiff of the Defendant.
WHEREFORE, Plaintiffdemands Judgment against the Defendant in the sum of $3297.23 with interest from March 28,
2017 together with the costs and disbursements of this action.
