BEFORE ME, the undersigned authority, personally appeared Keri-Anne Kolodiej (Affiant’), who being duly
sworn, deposes and says:
4 | am over the age of 18 years and am authorized to execute this Affidavit on behalf of Plaintiff pursuant to
corporate authority.
The information in this affidavit is taken from Plaintiff's business records. | have personal knowledge of
Plaintiffs procedures for creating and maintaining these records. They were: (a) made at or near the time of
the occurrence of the matters set forth therein by persons with personal knowledge of the information in the
business record, or from information transmitted by persons with personal knowledge; (b) kept in the course
of Plaintiff's regularly conducted business activities; and (c) created by Plaintiff as a regular practice.
Eunice V. Boykins Lawrence and Charles W. Barton ("Owners") own the following described real property
("Property"):
Unit Week 27, in Unit 2226, an Annual Unit Week in Vistana Cascades Condominium, pursuant to
the Declaration of Condominium as recorded in Official Records Book 5312, Page 2312, Public
Records of Orange County, Florida and all amendments thereof and supplements thereto

The Owners failed to pay assessments, accrued interest, and late charges due and payable pursuant to the
Declaration of Condominium related to the Property ("Delinquent Assessments").

A Claim of Lien for the Delinquent Assessments was recorded in the Official Records Book No. 10927, Page
7546 of the Public Records of Orange County, Florida.
The Plaintiff is the party entitled to enforce the Claim of Lien.
The amount due the Plaintiff as of November 9, 2017 is 2,604.14, which consists of the following:
Delinquent Assessments through the date of this Notice 724.35
Interest at 18% through November 9, 2017 321.10
Real Estate Taxes 141.39
Collection Costs or Other Fees Incurred 20.00
Late Charges 25.00
Title Search 45.00
Attorneys' Fees 500.00

interest beginning November 9, 2017 accrues at the rate of $0.36 per day.
