The undersigned, in accordance with Section 49.041 of the Florida Statutes, hereby deposes and states
under oath the following:
1 I am the attorney for Plaintiff in the above-styled cause, and am authorized to make this
Affidavit on behalf of the Plaintiff.
2 That diligent search and inquiry have been made to discover the residence of the Defendant and
whereabouts of all persons upon whom service would bind the Defendant and the same is set forth as more
particularly in the Return of Service and Affidavit of Diligent Search and Inquiry previously filed with the
ourt.
3 After making diligent search and inquiry, the last known residence of the Defendant is:

believes that there is no person in the state upon whom service of process would bind said absent or concealed
Defendant.
5 The age of the above-named Defendant is unknown but, Defendant is known to be over the age
of 18 years,
