                             Pursuant to Rule 4-7.21(g), Florida Rules of Professional Conduct, we hereby disclose that
                     the undersigned and all attorneys and staff of the Law Office of Sarah E. Blazak are employees of
                     GEICO General Insurance Company, a member of the GEICO group of insurance companies. The
                     undersigned is representing Defendant pursuant to a policy of insurance issued by GEICO General
                     Insurance Company, a member of the GEICO group of insurance companies.
                             Consistent with Rule 4-7.21(g) and the comments to that Rule, which recognize the public
                     policy of not disclosing the existence of insurance coverage to juries, the employment status of the
