Plaintiff, by its attorney Arthur Sanders, complains
of the defendant(s) as follows:
1. Plaintiff is a hospital at 150 Broad Street,
Hamilton, New York 13346.
2. The defendant, upon information and belief, is a
resident of the County of Cortland, residing at 3350 Pine
Hill Road, Marathon, New York.
3. That medical and/or surgical services were
rendered by physicians affiliated with the plaintiff to the
defendant on or about July 25, 2019 through July 26, 2019,
at defendant's express request.
4. That there remains due and owing a balance of
$14,736.30.

5. That plaintiff has demanded, in writing, payment
