PLEASE TAKE NOTICE, that upon the petition of GOVERNMENT EMPLOYEES
INSURANCE COMPANY ("GEICO") by its attorneys, Law Office of Gail S. Lauzon, the
Demand for Arbitration and all the pleadings and proceedings, a Motion will be made at the

Supreme Courthouse located at 360 Adams St. Brooldyn, NY 11201 on the day of December,
2018, at 9:30 a.m. in the forenoon of that day, or as soon thereafter as counsel can be heard for
an Order permanently staying the arbitration of this matter until such time as the respondent has
complied with all conditions precedent under the insurance contract, or in the alternative, an
Order directing respondent to provide all relevant medical records and authorizations, including
a no-fault authorization and to appear for a physical examination and an examination under oath
prior to proceeding to arbitration, together with such other and further relief as to this Court may
deem just and proper.
PLEASE TAKE FURTHER NOTICE, that pursuant to CPLR 2214(b), answering
affidavits must be served at least seven (7) days prior to the return date of this petition.
