penalties of perjury:
1. My name is Stephen P. Zaccheo, I reside at 7248 Bergen Court, Brooklyn, New
York, NY, 11234.
2. On August 1, 2018 I was involved in a motor vehicle accident at the intersection
of Linden boulevard and Van Siclen Avenue, Brooklyn, New York.
3. The police report filed for this accident indicates that there were cameras at the
location of my accident.
4. As a result of the accident, I sustained severe personal injuries.
5. My attorney informs me that they are attempting to preserve any video footage
that may have been captured by the careeras at the accident location.
6. My attorney informs me that this Court has the power to Order that such evidence
be preserved and turned over to my attorney's office.
