BEFORE ME, the undersigned authority, on this day personally appeared Norma Cortez,
Who, being by me first duly sworn did state under oath the following:
1. My name is Norma Cortez. I am over the age of eighteen (1 8),have never been convicted
0f a felony or a crime involving moral turpitude, am fully competent to make this affidavit,
and have personal knowledge of the facts stated herein Which are true and correct.
2. I am the Assistant Chief of the Human Resources Department of the Texas Department of
Public Safety (DPS). Ihave held this position since February, 2009. Personnel files for all
employees ofDPS are maintained in the Human Resources Department, and in my capacity
as records custodian, I have access to these personnel files, including payroll records.
Therefore, it is Within my personal knowledge Whether a person is 0r has been an employee
in the paid service 0f DPS at any given time.
3. Based 0n my review of the personnel files and payroll records maintained in the DPS
system, Human Resources Department, Ican confirm that Corporal Raymond Torres, III
has been a qu—time employee in the paid service of DPS from January, 2012 to the present.
