SUMMARY FINAL JUDGMENT a
THIS CAUSE, came on to be heard upon Plaintiffs Motion for Summary Final Trp,
Judgment. The Court finds that Emily A Madden was duly and properly served neh to
hearin 1g was WOLA on TS [Ie- Pldund Fagrpnec|, Defertant
Chapter 48, Florida Stamtes, he Court finds that there are no material facts in dispute and the Ai A ver
undisputed facts entitle boa to a judgment as a matter of law, and further that the Defendant, AYA
Emily A Madden, is indebted to Plaintiff Campus USA Credit Union the principal sum of
$25,954.63 for Count I and the principal sum of $3,375.02 for Count II, and prejudgment interest
of $1,585.99 for Count I and prejudgment interest in the amount of $219.69. The Court further
finds that Plaintiff is entitled to recover its taxable costs in the amount of $512.42.
Additionally the Court finds that pursuant to the agreement between the partics, Plaintiff
is entitled to the award of a reasonable attorney fee. The Court specifically finds, pursuant to the
affidavit of counsel in support of the award, and as required in Florida Paticnts Compensation
Fund v. Rowe, 472 So2d 1145 (Fla 1985), that counsel for Plaintiff reasonably expended 6 hours
of service for its client in this cause. Further, the Court finds the rate of $285.00 per hour to bea
reasonable fee as it is the prevailing rate for similar cases in this community, The Court also
determines the contingency risk factor in this case to be | resulting ina fee award in the amount
of $1,710.00.

oeIt is therefore ORDERED and ADJUDGED that Plaintiff, Campus USA Credit Union,
recover from the Defendant, Emily A Madden, whose Social Sccurity Number(s) is xxx-xx-0809,
the principal sum of $25,954.63 for Count I and the principal sum of $3,375.02 for Count II, and
prejudgment interest of $1,585.99 for Count I and prejudgment interest in the amount of $219.69,
plus costs herein taxed at $512.42, plus a reasonable attorney fee in the amount of $1,710.00, for
a total sum of $33,357.75, that shall bear interest at the rate of 4.84%, and as adjusted pursuant to
FS. 55.03, for all of which let execution issue.
itis further ORDERED and ADJUDGED that the judgment debtor(s) shall complete
under oath Florida Rule of Civil Procedure Form 1.977 (Fact Information Shect), including all
required attachments, and serve it on the judgment creditor's attorney, or the judgment creditor if
the judgment creditor is not represented by an attomey, within 45 days from the date of this final
judgment, unless the final judgment is satisfied or post-judgment discovery is stayed.
Jurisdiction of this case is retained to enter further orders that are proper to compel the
judgment debtor(s) to complete Form 1.977, including ali required attachments, and serve it on
the judgment creditor's attorney, or the judgment creditor if the judgment creditor is not
represented by an attorney.
The Clerk of this Court shall not record the Fact Information Sheet the Defendant(s)
is/are ordered to complete herein.
